Job Overview

Liberty Hospice offers a unique opportunity for compassionate individuals to make a meaningful difference in the lives of patients and families facing terminal illness. Our team provides state-of-the-art care and pain management services, delivered with empathy and respect.

We are currently seeking an experienced Hospice Aide (CNA) to join our team on a full-time basis.

Key Responsibilities
  • Provide direct patient care and support as part of our hospice services.
  • Maintain accurate and thorough documentation of patient care and progress.
Requirements
  • A high school diploma or equivalent, combined with successful completion of a Board of Nursing-approved training and competency evaluation program, current listing with the Division of Facility Services Registry, and at least one year of patient care experience in an acute care or custodial setting.
  • Demonstrated ability to organize work effectively and provide quality healthcare services to homebound patients through efficient patient care and timely documentation.
  • Capacity to travel and adapt to varying working conditions as assigned.
  • Familiarity with safe personal care practices and company policies, including effective communication skills and CPR certification.
  • Commitment to ongoing education and professional development, with a minimum of 12 hours of continuing education annually.
