Compassionate Palliative Care Specialist

We are seeking a skilled Palliative Care Nurse Practitioner to join our team at Lower Cape Fear LifeCare.
Job SummaryThis full-time position involves providing high-quality palliative care to patients in a hospital setting, working collaboratively with an interdisciplinary team to ensure comprehensive care.
Key Responsibilities:- Conduct patient assessments and develop personalized care plans, focusing on symptom management and quality of life.
- Educate patients' caregivers on disease management, medication administration, and end-of-life care.
- Collaborate with community medical providers and other disciplines to coordinate patient care.
- Participate in daily team meetings and contribute to complex case discussions.
- Work closely with inpatient hospice clinical liaisons to facilitate seamless care transitions.
We provide a supportive work environment, professional development opportunities, and a competitive compensation package. The salary for this role is approximately $120,000 - $140,000 per year, depending on experience.
Requirements:- A graduate degree in nursing with a specialization in palliative care or a related field.
- Current licensure as a nurse practitioner in the state of North Carolina.
- Experience working in palliative care or a related field, preferably in a hospital setting.
- Strong communication and interpersonal skills, with the ability to work effectively in a team environment.
