Job SummaryWe're seeking a skilled RN Case Manager to join our team at Abrazo Arrowhead Campus. As a key member of our clinical team, you'll play a critical role in ensuring seamless patient flow and optimal care outcomes.
Key Responsibilities- Conduct thorough assessments of patients' clinical needs and develop effective care plans to ensure timely and efficient discharge.
- Collaborate with interdisciplinary teams to coordinate patient care and resolve any issues that may impact patient flow.
- Utilize evidence-based criteria to review patients' clinical appropriateness and level of care, working with payers to authorize necessary treatments.
- Develop and maintain strong relationships with patients, families, and healthcare providers to ensure seamless communication and care coordination.
- Current Registered Nurse License in the State of Arizona.
- Minimum 3 years acute care hospital experience, with 1-2 years case management experience preferred.
- Strong knowledge of nursing process, continuum of care, and case management methods and standards.
- Excellent communication and documentation skills, with ability to work effectively in a fast-paced environment.
- 3 years experience in Case Management, Utilization Management, or Discharge Planning in acute care.
- Certified Case Manager (CCM) or Accredited Case Manager (ACM) certification.
