Position Title:
Manager of Care Coordination (CM)

Job Overview:

The Manager of Care Coordination (CM) is tasked with leading a team of healthcare professionals focused on optimizing member services, managing healthcare access, and ensuring effective care management. This role is pivotal in delivering comprehensive healthcare solutions tailored to the diverse needs of our member populations.

Key Responsibilities:
  • Oversee and manage a team dedicated to identifying and coordinating care for members with complex health conditions.
  • Facilitate service delivery through thorough assessments of both physical and mental health factors.
  • Collaborate with healthcare providers to set achievable short-term and long-term objectives that align with member needs and capabilities.
  • Identify members at risk for significant health complications.
  • Conduct reviews of benefit systems and perform cost-benefit analyses.
  • Assess medical, mental health, and substance abuse services for cost-effectiveness.
  • Support compliance with program standards and seek opportunities to enhance customer service and quality outcomes.
  • Contribute to quality improvement initiatives, ensuring adherence to industry standards and reporting requirements.
  • Recruit, train, mentor, and evaluate the performance of team members.
Qualifications:
  • Bachelor's degree and a minimum of 5 years of experience in Healthcare Management, or an equivalent combination of education and experience.
  • Active and unrestricted license as an RN, PA, LSW, LCSW, LPC, LMHC, or other recognized credentials as per state regulations.
  • Preferred qualifications include a Master's degree in Nursing, Public Health, Public Administration, Social Work, or Business Administration with a focus on Healthcare.
  • Certification as a Case Manager is preferred.
