Job Summary

We are seeking a highly skilled and dedicated Waiver Program Coordinator to join our team at the Massachusetts Rehabilitation Commission. As a Waiver Program Coordinator, you will play a critical role in supporting individuals with disabilities to live safely in the community.

Key Responsibilities
  • Coordinate an interdisciplinary team to develop individualized service plans (ISPs) for waiver participants, ensuring that their needs are met and they receive the necessary services and supports.
  • Conduct outreach and education to nursing facilities and rehabilitation hospitals to recruit waiver participants and inform staff about Home and Community Based Waivers.
  • Work collaboratively with clinicians to assess and determine strategies for addressing the individualized needs of waiver participants.
  • Develop and maintain relationships with waiver service providers and community resources to ensure that waiver participants receive the necessary services and supports.
  • Monitor and evaluate the effectiveness of waiver services and make recommendations for improvement.
Requirements
  • Ability to travel statewide is required.
  • Excellent communication and problem-solving skills.
  • Ability to work comfortably with remote technology.
  • Familiarity with the needs of individuals with disabilities living in the community.
Preferred Qualifications
  • Knowledge and experience working with individuals with disabilities and their families, preferably those with Acquired Brain Injury.
  • Knowledge of Medicaid, state agencies, and the provider service system and community-based resources available to serve persons with disabilities or elders.
