Advanced Patient Care Coordinator

The Clinical Nurse Coordinator (CNC) at Portsmouth Regional Hospital plays a vital role in delivering high-quality patient-centered care and coordinating all functions within the Intermediate Care Unit. This leadership position requires a skilled nurse who can partner with a multidisciplinary team to evaluate, customize, and deliver exceptional patient care.
Responsibilities:
- Partner with subject matter experts and leadership to support improved outcomes and services
- Provide coaching, feedback, and training to improve performance and engagement
- Assist with staff scheduling and admission/discharge processes
Qualifications:
- Advanced Cardiac Life Support certification required within 90 days of employment start date
- Basic Cardiac Life Support certification required within 30 days of employment start date
- Registered Nurse license in NH or compact state
- Associate Degree in Nursing or higher education
