Patient Care Coordinator

Cedar Healthcare Center is seeking an experienced Registered Nurse (RN) to fill the role of Nurse Practitioner.
Estimated Salary: $75,000 - $95,000 per year, depending on experience.
About the PositionThis role involves providing primary and specialty care to patients, as well as educating patients and families about health promotion and disease prevention. The successful candidate will have excellent clinical skills, with a strong commitment to delivering exceptional patient care.
Responsibilities:
- Provide direct patient care, including assessing patient needs, developing individualized care plans, and implementing evidence-based interventions.
- Collaborate with other healthcare professionals to ensure seamless transitions between departments.
- Educate patients and families about health promotion and disease prevention, with a focus on empowering individuals to take control of their health.
To be considered for this position, candidates must meet the following criteria:
- A current New Hampshire Registered Nurse (RN) License.
- Experience working in a healthcare setting, preferably in a long-term care or acute care environment.
- Advanced education in nursing, with a focus on nurse practitioner studies.
