Job Summary

We are seeking a Certified Recovery Specialist to join our Behavioral Health Department. The successful candidate will serve as a single point of contact for treating physicians, clinicians, and patients, providing resources and assistance with accessing clinical and supportive care services.

Key Responsibilities
  • Meet with patients at least monthly to assess compliance, address patient needs, and gain insight into unmet needs.
  • Coordinate information and patient needs with appropriate interdisciplinary providers.
  • Serve as an essential link between patients and all other care providers.
  • Interact with patient service representatives to expedite scheduling for follow-up and acute care appointments.
  • Facilitate appointments for consults and support services within established service standards.
  • Coordinate multidisciplinary team meetings.
  • Develop concise patient progress summaries for use by the care team.
  • Monitor and evaluate the delivery of care through documentation and tracking of needs addressed.
  • Follow patients throughout the care continuum.
  • Adhere to established productivity benchmarks.
  • Provide recovery education for every phase of the recovery journey.
  • Provide a model for both people in recovery and staff by demonstrating that recovery is possible.
  • Assist recovering persons to identify their personal interests, goals, strengths, and weaknesses regarding recovery.
  • Assist/coach recovering persons to develop their own plan for advancing their recovery.
  • Actively identify and support linkages to community resources that support the recovering person's goals and interests.
  • Support connections to community-based, mutual self-help groups.
  • Link individuals to appropriate professional resources.
  • Provide vision-driven hope and encouragement for opportunities at varying levels of involvement in community-based activities.
  • Develop relationships with community groups/agencies in partnership with others in the agency.
  • Identify barriers to full participation in the COE and/or community resources and develop strategies to overcome those barriers.
  • Maintain contact with recovering persons for 12 months once they engage in the COE to ensure ongoing success and to provide re-engagement support if needed.
Requirements
  • CPS or CRS certification is required.
  • Must maintain valid CRS certification and remain in good standing during employment.
  • High School Diploma/GED is required.
  • Education Specialization: Behavioral Health, Nursing, or related field of study is required.
