Outreach Case Manager

Special Service for Groups, Inc. - HOPICS is seeking a highly motivated and compassionate Outreach Case Manager to join our team. As a key member of our multidisciplinary team, you will provide professional case management services to homeless consumers, linking them to essential benefits, housing supports, and other mental health, physical health, and substance use programs.
Key Responsibilities:- Conduct immediate needs assessments with adult consumers to develop comprehensive treatment plans.
- Collaborate with internal and external entities as part of a multidisciplinary team.
- Provide assistance to homeless clients in the field, establishing rapport and building trust.
- Coordinate treatment plans with therapists, nurses, and substance use counselors.
- Assist consumers in resolving or coping with mental, emotional, medical, and/or substance-related problems.
- Document all notes and treatment plans in accordance with established guidelines.
- Some college credits or at least two years of experience working with homeless individuals.
- Ability to work with diverse cultural, ethnic, and socio-economic backgrounds.
- Effective communication and interpersonal skills.
- Neat and professional appearance.
- Attention to detail and punctuality.
- Working knowledge of Microsoft Word, Excel, and Internet.
This position requires working in various environments, including homeless encampments, and interacting with homeless individuals, individuals with severe physical and mental disabilities, and individuals with infectious diseases. The incumbent must be able to work effectively under pressure and deal with traumas that may trigger personal trauma.
