About the Role

We are seeking a highly skilled MDS Coordinator to join our team at Sun Health. As a key member of our healthcare team, you will play a critical role in ensuring the delivery of high-quality care to our residents.

Key Responsibilities
  • Resident Assessment and Care Planning
    • Conduct and coordinate the development and completion of the resident Minimum Data Set (MDS) assessment in accordance with current rules, regulations, and guidelines.
    • Develop and implement comprehensive care plans that address the unique needs and goals of each resident.
  • Quality Assurance and Compliance
    • Develop and maintain an ongoing quality assurance program for the Resident assessment/care plans.
    • Ensure compliance with state and federal regulations, as well as internal policies and procedures.
  • Administrative and Support Functions
    • Perform administrative duties, such as completing medical forms, reports, evaluations, and studies as necessary.
    • Assist the Director of Social Services and the Admissions Coordinator in completing the care plan portion of the Resident's discharge plan.
  • Collaboration and Communication
    • Work collaboratively with the Resident Care Plan team to develop comprehensive Resident assessments and care plans.
    • Communicate effectively with residents, families, and healthcare professionals to ensure seamless care coordination.
Requirements
  • A current active license as a Licensed Practical Nurse or Registered Nurse in the State of Arizona.
  • Knowledge of nursing and medical practices and procedures, as well as laws, regulations, and guidelines that pertain to long-term care.
  • MDS Coordinator experience within the recent 5 years.
  • Possessing a current or ability to obtain a Level 1 Fingerprint Clearance Card.
