Care Management RN Manager

We are seeking a highly skilled and experienced Care Management RN Manager to join our team at Providence Portland Medical Center. As a key member of our leadership team, you will be responsible for the direction and management of the inpatient Care Management Department.
Key Responsibilities- Provide leadership and oversight for the clinical care management model
- Manage a team of approximately 24-31 FTE staff
- Oversee the operations of the care management department
- Collaborate with all other disciplines to advance the clinical care management model and improve coordination of patient care
- Bachelor's Degree in Nursing
- Oregon Registered Nurse License
- 2 years Supervisory experience, including program planning, performance management, staff development and evaluation
- 1 year Care Management experience
- 3 years Relevant experience in one or more of the following areas: Social Services, Utilization Review/Quality Management, Case Management
- Master's Degree in Nursing
