About the Role

The RN Housing Care Manager is a key member of our Elderplace team, responsible for overseeing and coordinating the care and services for at-risk frail elder patients with complex medical and psychosocial needs.

Key Responsibilities
  • Provide professional nursing care for residents of ElderPlace program housing, consistent with the Oregon Nurse Practice Act and Oregon Administrative Rules and Regulations for Assisted Living and Residential Care Facilities.
  • Apply knowledge based on the nursing process while performing assessments and evaluating for changes of condition.
  • Teach and/or delegate nursing tasks to unlicensed caregivers/Personal Care Aides (PCAs).
  • Participate in the weekly IDT team meeting as well as various Housing team meetings.
Requirements
  • Oregon Registered Nurse License upon hire
  • National Provider BLS - American Heart Association within 30 days of hire
  • Relevant certifications applicable to the position, as specified by the Nurse Manager upon hire
  • 1 year ElderPlace experience
  • Experienced within Specialty: RN experience in a PACE organization
