About the Role

The RN Housing Care Manager plays a vital role in overseeing and coordinating the care and services for at-risk frail elder patients with complex medical and psychosocial needs. This position requires a broad range of general nursing knowledge, skills, and expertise to provide care that is appropriate and personalized.

Key Responsibilities
  • Provide professional nursing care for residents of ElderPlace program housing, consistent with the Oregon Nurse Practice Act and Oregon Administrative Rules and Regulations for Assisted Living and Residential Care Facilities.
  • Participate in the weekly IDT team meeting as well as various Housing team meetings to ensure effective team functioning and reflect the mission and philosophy of the Sisters of Providence.
  • Teach and/or delegate nursing tasks to unlicensed caregivers/Personal Care Aides (PCAs) to promote effective care delivery.
  • Perform all duties in a manner that promotes effective team functioning and reflects the mission and philosophy of the Sisters of Providence.
Requirements
  • Oregon Registered Nurse License upon hire
  • National Provider BLS - American Heart Association within 30 days of hire
  • Relevant certifications applicable to the position, as specified by the Nurse Manager upon hire
  • 1 year ElderPlace experience
Preferred Qualifications
  • Bachelor's Degree in Nursing
  • 1 year Acute care RN experience
  • 2 years ElderPlace experience
