Job Title: Clinical Diabetes Specialist

Kaleida Health is seeking a skilled Clinical Diabetes Specialist to join our team. As a member of our multidisciplinary team, you will play a vital role in providing exceptional patient care and education.

Salary: $42.09-$53.86 per hour *Wage will be determined based on factors such as candidate's experience, qualifications, internal equity, and any applicable collective bargaining agreement.*

About the Job:

As a Clinical Diabetes Specialist, you will work closely with patients, families, and healthcare teams to develop and implement individualized care plans that address the unique needs of each patient. Your expertise will include assessing and treating human responses to actual and potential health problems through case finding, health teaching, health counseling, and provision of supportive or restorative care.

The successful candidate will have a strong background in nursing practice, with a focus on diabetes management and education. You will also possess excellent communication and interpersonal skills, allowing you to effectively collaborate with interdisciplinary teams and provide compassionate patient care.

Requirements:

A Bachelor of Science degree in Nursing is required for this position. Additionally, you must hold current NYS registration as a Registered Professional Nurse. Certification as a Certified Diabetes Educator (CDE) is preferred but not mandatory. If you are an internal applicant, you must have been employed in a comparable area of practice for at least nine continuous months within two years of the job posting. External applicants require one year of comparable work experience within two years of the job posting. All applicants must satisfy all orientation/probationary requirements.
