The Physician position is central to our collaborative care model. As a value-driven healthcare provider, we prioritize quality outcomes for our patients. This model allows our Physicians to manage a smaller patient load, ensuring they can devote more time to each individual.
Personalized Patient Care:
At Bloom Healthcare, we recognize that each patient has distinct experiences and health objectives. We customize each care plan to align with these goals, fostering meaningful relationships between our clinicians and patients.
What We Offer:
- Participation in our employee equity program
- Competitive base salary of $225,000
- Quality bonus potential of up to $35,000

- No primary call responsibilities

- Defined service area near your residence
- Flexible scheduling options (3, 4, or 5-day work weeks)
- Manage a maximum of 10 patient visits per day
Key Responsibilities:
- Provide comprehensive primary care through home visits for up to 10 patients daily
- Conduct data-informed, condition-specific specialty visits as necessary
- Oversee health outcomes for an assigned patient population
- Collaborate with a team of advanced practice providers
- Exhibit exceptional communication and relationship-building skills with team members and external partners
- Utilize population health data to enhance patient outcomes
- Participate in ongoing educational initiatives
- Travel to designated assisted living facilities for patient care
