The Physician position is central to our collaborative care model. As a value-driven healthcare provider, we prioritize quality outcomes for our patients. Our dedicated primary care team includes physicians, advanced practice professionals, and a supportive administrative staff. This model allows our Physicians to manage a smaller patient load, ensuring they can devote more time to each individual.

Personalized Patient Care:

  • Manage a maximum of 10 patient visits per day

Key Responsibilities:

  • Provide comprehensive primary care through home visits for up to 10 patients daily
  • Conduct data-informed, condition-specific specialty visits as necessary
  • Oversee health outcomes for an assigned patient population
  • Collaborate with a team of advanced practice providers
  • Exhibit exceptional communication and relationship-building skills with team members and external partners
  • Utilize population health data to enhance patient outcomes
  • Participate in ongoing educational initiatives
  • Travel to designated assisted living facilities for patient care
