The Physician position is pivotal within our collaborative care model. As a value-driven healthcare provider, we prioritize quality outcomes for our patients. Our primary care team includes physicians, advanced practice professionals, and dedicated administrative staff, all supported by a range of resources including social work, pharmacy, behavioral health, and integrated hospice services. This comprehensive approach allows our Physicians to focus on fewer patients each day, ensuring they can dedicate more time to each individual.

Personalized Patient Care:

At Bloom Healthcare, we recognize that each patient has distinct experiences and health goals. We customize care plans to align with these objectives, fostering meaningful relationships between our clinicians and patients.

What We Offer:

  • Manageable patient load of 10 visits per day

Key Responsibilities:

  • Provide comprehensive primary care through home visits to 10 patients daily
  • Conduct data-informed, condition-specific specialty visits as required
  • Oversee health outcomes for an assigned patient population
  • Coordinate care with a team of advanced practice providers
  • Exhibit outstanding communication and relationship-building skills with team members and external partners
  • Utilize population health data to enhance patient outcomes
  • Participate in ongoing educational opportunities
  • Travel to designated assisted living communities for patient care
