Patient Navigator

Sun River Health is seeking a highly skilled Patient Navigator/Patient Representative to join our team at our White Plains, NY location. As a Patient Navigator/Patient Representative, you will play a vital role in providing patient-centered care and facilitating access to services in a pleasant, knowledgeable, and efficient manner.
Key Responsibilities:- Assist with daily reception activities and ensure that all processes needed for patient services are initiated.
- Communicate effectively with team members and patients regarding all aspects of service.
- Responsible for telephone and mail contact with patients.
- Facilitate patient care within different departments of Sun River Health and outside agencies.
- Refer patients to appropriate agencies, with a focus on referrals to specialty care and public health insurance programs, as needed and appropriate.
- Consult with medical staff to link patients to services through the provision of ancillaries such as translation, transportation assistance, and application assistance.
- Responsible for all necessary clinical information available to specialists for referral groups; track referral appointments and obtain consultant reports; obtain consent for release of information.
- Assist patients in overcoming obstacles that prevent them from receiving needed referrals.
- Provide basic instruction/health education to clarify provider instructions, procedures, and referral needs.
- Responsible for the scheduling of appointments, fee collection, explanation of services and charges for patient services.
- Verify all insurance and set fees based on the sliding scale fee.
- High School diploma/GED
- 2-4 years of work-related experience
- Preferred language: Bilingual in both English and Spanish (orally and written)
