Role Summary:

We are seeking a compassionate Care Coordinator to develop tailored care strategies for patients. The Care Coordinator will conduct initial assessments upon patient admission to guarantee the provision of suitable support services. This role involves ensuring that all healthcare professionals involved are informed with the latest case details and working collaboratively with patients' families and caregivers to effectively disseminate information. The Care Coordinator will also build a network of quality service providers and maintain precise patient documentation. You will connect patients with community resources and benefits, including housing assistance and self-management programs.

To excel in this position, you will navigate patients through the healthcare landscape and foster relationships with healthcare professionals. Ideal candidates will be adept at multitasking in a fast-paced environment and possess strong negotiation abilities.

Key Responsibilities:

  • Conduct comprehensive initial evaluations and create/update individualized care plans for patients.
  • Manage all aspects of multidisciplinary care coordination.
  • Communicate and share relevant patient information with appropriate healthcare team members to facilitate treatment.
  • Engage with patients' families, friends, and support systems in formulating treatment strategies.
  • Propose alternative treatment options when patients' service requests do not meet medical necessity standards.
  • Educate patients on procedures, healthcare provider directives, and referrals.
  • Connect patients to social service programs such as transportation assistance and language services.
  • Perform regular follow-ups with patients to assess progress, promote continuity of care, and ensure enhanced health outcomes.
  • Adhere to our organization's performance standards, policies, and expected behaviors.
  • Execute other related duties as assigned.

Qualifications:

  • Current RN licensure in the state of practice.
  • Bachelor's degree from an accredited nursing program required.
  • A minimum of five (5) years of clinical/hospital experience in Critical Care or Medical-Surgical nursing.
  • Certification as a Case Manager (CCM) is preferred.
  • Prior experience in case management is preferred.
  • Familiarity with healthcare financial, regulatory, and payer issues is preferred.
  • Knowledge of state, local, and federal programs is strongly preferred.
  • PRI/Screen certification is strongly preferred.
  • Proficient in Healthcare Management Systems and Microsoft Office Suite (Word and Excel).
  • Exceptional communication skills, both written and verbal.
  • Strong organizational and negotiation skills.
