About the Role

The Home Healthcare RN Director - Patient Care Coordination is responsible for leading the clinical operations of our home health hospice program. As a key member of the leadership team, you will ensure that patients receive high-quality care while maintaining efficient operations.

Key Responsibilities

  • Oversee daily clinical and operational functions, ensuring alignment with agency policies and standards
  • Supervise and evaluate patient care services, providing guidance and support to staff members
  • Liaise with referral sources, including family members, to provide explanation of hospice services and ensure safe transitions
  • Act as a liaison between the Intake office and Patient Care Coordinators and their teams, facilitating seamless coordination of patient care

Qualifications

  • Bachelor's degree in Nursing from an accredited institution required; Master's degree in a health-related field preferred
  • At least six years of professional nursing experience, including two years in community settings, with at least two years of supervisory experience
  • Active NYS RN license and valid driver's license with insurance
  • Excellent communication and interpersonal skills

Benefits

This position offers a competitive salary range of $160,000 - $180,000 plus an attractive benefits package, including paid time off, pension, participation in 403(b), and optional benefits like short-term disability, life insurance, and cancer insurance.
