Salary and Benefits

The salary for this position is $70,000 - $82,000 per year, plus a $15,000 sign-on bonus. Our company offers a comprehensive benefits package, including health insurance, retirement plans, and paid time off.

Job Responsibilities

This field-based job requires providing psychosocial support and social work services to Hospice patients and their families in various settings, including inpatient units, home care, and nursing homes. As a key member of the hospice interdisciplinary group (IDG), you will develop social work plans of care, goals, and recommendations based on patient/family needs.

Requirements
  • Graduate from an accredited School of Social Work
  • Minimum of one year experience in Social Work in a healthcare setting
  • Knowledge of Word and Excel preferred
  • New York State MSW licensure
  • Valid NYS Driver's license and car required
  • Ability to work within an interdisciplinary team environment
  • Possess excellent organizational, communication, and listening skills
  • Ability to input clinical data proficiently
  • Palliative Care Certificate preferred
  • Pediatric Specialty a plus
  • Bilingual English/Spanish preferred
