Key Responsibilities- Provide Evidence-Based Case Management Services: Utilize evidence-based practices to support residents in achieving their goals and maintaining stable housing.
- Complete Intake and Assessments: Conduct thorough intake and assessments to identify residents' needs and develop personalized plans.
- Implement Housing First and Harm Reduction Interventions: Work with residents to develop and implement plans that prioritize housing stability and address underlying issues.
- Assist Residents in Obtaining Stable Sources of Income: Help residents access resources and services to secure stable income and achieve financial stability.
- Education: Bachelor's degree in human services or a related field required; Master's degree preferred.
- Experience: 1 year of case management experience required; 3 years of experience working with diverse populations preferred.
- Skills: Proficiency in Microsoft Office and ability to work effectively with vulnerable adults.
