Your Impact:

As a Senior Social Services Coordinator, you will play a crucial role in coordinating external psychiatric and psychosocial services to support behavior modification initiatives, ensuring holistic care for residents. Your responsibilities will include assessing resident needs through social service interviews, providing counseling, advocacy, discharge planning, and facilitating access to community resources.

You will encourage resident engagement in both individual and group activities, fostering participation within and outside the facility. Additionally, you will promote family involvement in care plans and assist residents with legal documentation, ensuring smooth transitions and ongoing family support.

Meticulous documentation of social service plans and progress notes in resident records will be essential, accurately reflecting goals and interventions.

Schedule: This is a full-time position, Monday through Friday, from 8 AM to 4 PM, with occasional weekend rotation.

Qualifications:

  • Preferred experience in geriatrics.
  • Experience working with diverse racial, ethnic, and socioeconomic groups is advantageous.
  • Relevant experience and/or training, or an equivalent combination of education and experience.
  • Preferred minimum of 3 years of social services experience in a Long-Term Care environment.
