Job Summary

We are seeking a highly compassionate and empathetic Patient Liaison Specialist to join our team. As a key member of our care team, you will play a vital role in identifying and addressing the social determinants of health that impact our patients' well-being. Your primary responsibility will be to work closely with patients, clinicians, and community partners to develop personalized care plans that promote health equity and improve patient outcomes.

Key Responsibilities
  • Conduct thorough assessments of patients' social needs and develop targeted care plans to address identified gaps
  • Collaborate with patients, clinicians, and community partners to ensure seamless care coordination and communication
  • Research and identify community resources that support patients' social determinants of health
  • Develop and maintain relationships with community partners to enhance care delivery and patient outcomes
  • Provide education and support to patients and their families on health-related topics and community resources
Requirements
  • Minimum of one year of experience working in medical settings, community-based organizations, or related service industries
  • Knowledge of medical terminology and healthcare systems
  • Excellent communication and interpersonal skills, with the ability to work effectively with diverse populations
  • Ability to work independently and as part of a team, with a strong focus on patient-centered care
  • High school diploma or equivalent required; associate's or bachelor's degree preferred
Preferred Qualifications
  • Experience working with seniors or individuals with complex social needs
  • Knowledge of community resources and social services
  • Ability to speak multiple languages, including Mandarin, Cantonese, or Vietnamese
  • Experience working in a primary care office or community-based organization
