Care Coordinator

Bay State Community Services is seeking a highly organized and compassionate Care Coordinator to join our team. As a Care Coordinator, you will work collaboratively with patients, families, and healthcare providers to coordinate care management services and ensure that individuals receive the necessary support and resources to achieve their health and wellness goals.
Key Responsibilities:- Work collaboratively with care management teams, patients, and families to develop and implement care plans that meet individual needs and goals.
- Monitor patient progress, evaluate the effectiveness of care plans, and make adjustments as needed.
- Facilitate patient access to medical and specialty providers, as well as community resources and services.
- Provide education and support to patients and families on relevant health and wellness topics.
- Cultivate and support primary care and specialty provider co-management through timely communication and integration of information into care plans.
- 1 year of experience working with mental health/substance use disorders, with a Bachelor's degree in a social services field; or 2 years of experience with an Associate's degree or no degree.
- Strong computer skills and experience with EMR systems and reports.
- Highly organized with the ability to keep accurate notes and records.
- Local knowledge of community health care and social welfare resources is desirable.
