Job Title: Care Coordinator

Bay State Community Services is seeking a highly organized and detail-oriented Care Coordinator to join our team. As a Care Coordinator, you will work collaboratively with patients, families, and healthcare providers to coordinate care management services and ensure seamless transitions in care.

Key Responsibilities:
  • Work collaboratively with care management teams, patients, families, and healthcare providers to coordinate care management services.
  • Monitor adherence to care plans, evaluate effectiveness, and facilitate changes as needed.
  • Create ongoing processes for patients and families to determine and request the level of care coordination to support their needs.
  • Ensure care plans meet the requirements of EOHHS and notify the ACO/MCO of any changes.
  • Facilitate patient access to medical and specialty providers, educate patients and families about community resources, and facilitate meetings between patients, families, and care teams.
  • Ensure patients receive necessary assistance and accommodations to prepare for and participate in treatment planning.
  • Cultivate and support primary care and specialty provider co-management through timely communication, inquiry, follow-up, and integration of information into care plans.
  • Assist with the identification of high-risk enrollees and flag in the EMR.
  • Conduct assessments for Flexible Services and make recommendations to the ACO for approval.
  • Provide health and wellness coaching as directed by the care team and as indicated in the care plan.
  • Maintain regular contact with patients to monitor and coordinate care planning, including quarterly face-to-face meetings.
Requirements:
  • 1 year of experience working with mental health/substance use disorders with a Bachelor's degree in a social services field; or 2 years of experience working with mental health/substance use disorders with an Associate's degree or no degree.
  • Strong computer skills and experience with EMR systems and reports is desirable.
  • Highly organized with the ability to keep accurate notes and records.
  • Local knowledge about and connections to community health care and social welfare resources is desirable.
  • Bilingual speakers (Haitian Creole, Spanish, Portuguese, Cape Verdean Creole, etc.) are encouraged to apply.
