Job Summary

As a Community Health Worker, the PASO Resource Specialist plans, develops, markets, and delivers public health education to local health program participants at Prisma Health Upstate and community groups. Facilitates enrollment of eligible participants in Medicaid, WIC and SNAP Programs. The Resource Specialist position is based out of the PASOs Greenville office at the Center for Pediatric Medicine. In addition to educating individuals/groups, the Resource Specialist supports the PASO Program Manager in outreach activities, recruitment and resource navigation.

Key Responsibilities

  • The Resource Specialist will focus education and enrollment efforts in the designated area of South Carolina.
  • Travels to areas where there are large Latino populations to offer information and enrollment services, collaborating with local PASO Coordinators to connect with community groups.
  • Delivers educational sessions to 50 people per month, for a total of 600 individuals per year, offering insurance enrollment support and other services.
  • Assists the PASO Program by organizing at least 1 community outreach event per month where both PASOs and ACA information is offered.
  • Provides assistance in helping individuals connect to needed resources to reach PASO target goals for the designated area.
  • Supports Inpatient/Outpatient Social Work and Case Management staff as they work with Spanish Speaking Hispanic/Latino patients.
  • Collects data to report on program outcomes.

Requirements

  • Bachelor's Degree in Health Education, Health Promotion, or a closely related field.
  • 1 year of experience in Health Education.
  • Bilingual in Spanish and English.
  • Ability to help enroll children and families in Medicaid and educate about ACA options.
  • Knowledge of the principles, practices, and theories of ACA, health promotion, education, and advocacy.
