As part of our Senior Community Care (SCC) product, you'll work with a team to provide expert care to patients in their homes, nursing facilities, assisted living, and senior housing.

About the Role

This rewarding position offers a unique blend of clinical expertise and coordination skills, enabling you to make a tangible difference in patients' lives. As an Advanced Practice Care Coordinator, you will:

  • Deliver cost-effective, high-quality care to assigned members, managing medical and behavioral conditions effectively.
  • Perform comprehensive assessments, document findings, and ensure accurate diagnoses and ICD-10 coding.
  • Facilitate agreement and implementation of member plans of care, engaging facility staff, families, primary and specialty care physicians.

Requirements

To succeed in this role, you'll need:

  • Active national NP or PA certification or the ability to obtain national certification in the state of assignment by start date.
  • Active and unrestricted APC license in the State of New Hampshire.
  • Current active DEA licensure/prescriptive authority or ability to obtain post-hire, per state regulations (unless prohibited in state of practice).
  • Access to reliable transportation for travel to client and/or patient sites within a designated area.
