Quality Improvement Coordinator RN

At NHC HomeCare Midlands, we are seeking a highly skilled and dedicated Quality Improvement Coordinator RN to join our team. As a Quality Improvement Coordinator RN, you will play a critical role in ensuring accurate assessments and effective plans of care for our patients.

Responsibilities:
  • Conduct thorough and accurate assessments to develop effective plans of care.
  • Ensure correct and educated coding according to official guidelines and as approved by the assessing clinician.
  • Facilitate, coordinate, and monitor Quality Assessment Performance Improvement initiatives regarding outcomes management.
  • Aggregate and report compliance monitors and aberrancies.
Requirements:
  • Unencumbered current Registered Nurse license in a provider state.
  • Deep knowledge and understanding with ongoing education in home health regulations, processes, and outcomes.
  • BSN preferred.
  • Coding certification preferred.
  • 1 year experience in home health care.
