Job Title: Certified Peer Support Specialist

At Southeastern Integrated Care LLC, we are seeking a highly motivated and compassionate Certified Peer Support Specialist to join our team. As a Peer Support Specialist, you will play a vital role in providing coaching, mentoring, and consultation to individuals in recovery, promoting self-advocacy, self-direction, and wellness.

Key Responsibilities:
  • Provide individualized coaching and mentoring to clients, focusing on recovery, self-advocacy, and self-direction.
  • Collaborate with interdisciplinary healthcare teams to promote a culture of recovery and empowerment.
  • Develop and implement personalized recovery plans, incorporating client goals and preferences.
  • Provide linkage to community resources, including education, employment, and housing services.
  • Document direct service delivery, including purpose of contact, interventions, time spent, and effectiveness.
Requirements:
  • High School Diploma/GED
  • NC Certified Peer Support Specialist certification
  • Valid driver's license
  • Ability to read medical records, write service notes, and present information effectively
Work Environment:

The work environment is primarily in the client's living environment and in the community, with occasional exposure to domestic animals, pests, and other environmental factors.

Physical Demands:

The physical demands of this role include standing, walking, reaching, and lifting up to 20 pounds, with occasional lifting up to 50 pounds.
