Job Description:

Care Coordination

We are seeking an experienced Hospice Registered Nurse (RN) to join our team. As a Hospice Nurse Care Coordinator, you will be responsible for providing high-quality patient care and coordination of services with our interdisciplinary team.

  • Develop and implement individualized care plans that meet the unique needs of each patient
  • Coordinate with patients, families, physicians, and other healthcare providers to ensure seamless care transitions
  • Provide education and support to patients and families to promote independence and quality of life
Requirements

To be successful in this role, you will need:

  • Current RN licensure in the state of practice
  • At least two years of clinical nursing experience, with at least one year of hospice nursing experience preferred
  • Knowledge of home health and hospice care programs, including reimbursement guidelines
  • Strong communication and interpersonal skills
