Remote Care Coordinator

ARcare Inc is seeking a dedicated Remote Care Coordinator to join our team. As a Remote Care Coordinator, you will be responsible for coordinating medical care and disease management activities for patients with chronic diseases. This is a challenging yet rewarding position that requires strong communication, organizational, and problem-solving skills.
Key Responsibilities:
- Coordinate medical care and disease management activities, including referrals for medical evaluation and treatment.
- Develop and implement individualized medical services plans for patients with chronic diseases.
- Conduct regular case conferencing with patients' healthcare providers to ensure seamless transitions of care.
- Provide education and support to patients on healthy living habits, disease management, and medication adherence.
Requirements:
- CURRENT LPN LICENSE IN APPROPRIATE STATE REQUIRED
- At least one year of experience in a medical facility
- Excellent communication, organizational, and problem-solving skills
- Able to work effectively in a fast-paced environment
Estimated Salary: $50,000 - $60,000 per annum (dependent on location and experience)
