Medical Care Coordinator

EMrecruits is seeking a skilled Medical Care Coordinator to join their team in Fayetteville, North Carolina. As a key member of our healthcare team, you will be responsible for coordinating patient care and ensuring seamless transitions from hospital to home.
Estimated Salary: $65,000 - $80,000 per year
Key Responsibilities- Collaborate with healthcare providers to develop individualized care plans
- Coordinate with community services to ensure patients receive necessary support
- Maintain accurate records and communicate effectively with patients and families
- Act as a liaison between patients, providers, and community agencies
- Ensure continuity of care and facilitate timely follow-up appointments
- Educate patients on disease management and medication adherence
- Advocate for patients' needs and preferences
- Audit charts and compile data to support quality improvement initiatives
- Degree in Nursing (RN or LPN) or RMA
- Experience in direct patient care or managed care preferred
- Valid driver's license and personal vehicle required for travel
- HEDIS Quality Measures experience and proficiency in Microsoft Office
- Excellent communication and customer service skills
- Ability to work independently and collaborate with teams
