About the Role

La Frontera Arizona is seeking a highly skilled Crisis Transition Navigator to join our team. As a Crisis Transition Navigator, you will play a critical role in supporting individuals who have received crisis services and are in need of post-crisis assistance.

Key Responsibilities
  • Meet with consumers of crisis services in the community to assess their needs and develop a Crisis Navigation Plan (CNP).
  • Collaborate with community-based resources and RBHA providers to ensure seamless transitions and access to necessary services.
  • Provide short-term recovery coaching and support to individuals, helping them develop and implement self-directed recovery plans.
  • Maintain accurate and thorough documentation of client progress and CNPs.
  • Develop and maintain relationships with community-based resources and RBHA providers to ensure access to necessary services.
Requirements
  • High School Diploma or GED.
  • Peer Certification and/or experience preferred.
  • Substance Abuse experience preferred.
  • Must self-identify as a recipient of mental health services or having assisted a family member in their mental health or substance abuse recovery process.
  • Identifies as a Military Veteran, Service Member, or Service Family Member preferred.
  • Effective interpersonal skills and ability to work with diverse populations.
  • Two years of prior experience preferred.
