  • Client Assessment and Support
    • Conduct thorough assessments and interviews with clients to determine eligibility for our services.
    • Develop and update personalized action plans, case notes, and other documents related to a client's file.
  • Life Skills Development
    • Provide ongoing life skills development to assist clients in improving independence and quality of life.
    • Identify and address specific needs and challenges faced by clients.
  • Community Resource Connection
    • Connect clients to medical, mental health, and other supportive counseling services.
    • Establish a network of community services and conduct appropriate referrals to clients.
  • Benefits Navigation
    • Guide clients through the process of application, qualification, re-determination for SSI, SSDI, Medicaid, Medicare, SNAP, and other services as needed.
  • Communication and Collaboration
    • Attend and actively participate in scheduled internal and external meetings.
    • Represent Compass at professional meetings, community events, and to members of the community.
Requirements
  • Education and Experience
    • High school diploma or general education degree (GED) with preference for a Bachelor's degree in the social service field.
    • 2 years experience in social service field.
    • One-year related experience and/or training working with people in crisis, specifically in housing and social services.
  • Skills and Abilities
    • A valid driver's license, insurance, and own vehicle transportation.
    • Ability to perform CPR/First Aide as needed.
    • Ability to lift up to 50 lbs.
