Home Health Licensed Practical Nurse

Pinnacle Home Care is seeking a skilled and compassionate Licensed Practical Nurse (LPN) to join our team of healthcare professionals in the Pasco County area. As an LPN with Pinnacle Home Care, you will have the opportunity to provide high-quality patient care in the comfort of their own homes.
Key Responsibilities- Provide Skilled Nursing Care: Under the direct supervision of a Registered Nurse, you will perform selected skilled actions in the provision of curative, rehabilitative, palliative, or preventative nursing care.
- Assist with Patient Care Planning: You will assist with the preparation, implementation, and continuing evaluation of the client care plan, ensuring that patients receive individualized and effective care.
- Communicate Effectively: You will communicate effectively with patients, families, and healthcare team members to ensure seamless care coordination and patient satisfaction.
- Monitor Patient Progress: You will observe, record, and report patient symptoms, reactions, and changes to the healthcare team, ensuring that patients receive timely and appropriate interventions.
- Participate in Quality Improvement: You will actively participate in quality improvement initiatives, including the Agency's QAPI program, to ensure that patient care meets the highest standards.
- IV Certification: You must be certified in IV therapy and have the ability to perform IV-related procedures.
- Effective Communication: You must be able to communicate effectively with patients, families, and healthcare team members.
- Physical Demands: You must be able to access client homes, which may not be routinely wheelchair accessible, and perform physical assessments and client care activities.
- Education and Licensure: You must be a graduate of an accredited School of Practical Nursing and be licensed as a Licensed Practical Nurse in the state of Florida.
- Experience: You must have one or more years of experience in an acute/sub-acute care facility, with home health experience preferred.
