JOB SUMMARY:
The Licensed Practical Nurse is accountable for delivering skilled nursing care under the supervision of a Registered Nurse. Responsibilities include executing specific skilled actions in the provision of curative, rehabilitative, palliative, or preventive nursing care. The LPN is responsible for making informed decisions based on their nursing expertise and educational background.

KEY RESPONSIBILITIES:
  • Implement the care plan under the guidance of the registered nurse.
  • Assist the Registered Nurse in executing specialized procedures as instructed under physician's orders.
  • Contribute to the preparation, execution, and ongoing assessment of the client care plan.
  • Recognize and address the social and economic factors affecting clients, ensuring their emotional and physical comfort and safety.
  • Educate patients on appropriate self-care techniques.
  • Encourage teamwork among staff by understanding the roles of others involved in client care and actively participating in team meetings.
  • Commit to personal and professional growth through active involvement in nursing organizations and participation in training programs.
  • Document clinical and progress notes, including interim orders with measurable goals and timelines, integrating these into the patient's care plan under RN supervision.
  • Monitor, document, and report patient symptoms, reactions, and changes to the appropriate personnel.
  • Facilitate relationships between clients, staff, and their families.
  • Assist in client rehabilitation according to the medical care plan, promoting client engagement and self-sufficiency.
  • Report any abnormalities in clients' physical condition, mobility, or alignment.
  • Participate in nursing organizations and contribute to in-service training programs.
  • Be aware of the Agency's Quality Assurance and Performance Improvement (QAPI) program and participate as required.
  • Manage and follow up on client complaints in accordance with the Agency's policies.
  • Understand patient rights and the Agency's obligations to uphold these rights.
  • Recognize appropriate reasons for patient discharge and the Agency's policies regarding discharge.
  • Identify potential signs of abuse and neglect and know the procedures for reporting.
  • Develop patient-centered and individualized goals with realistic, measurable timelines under RN supervision.
  • Implement infection prevention and control measures in accordance with Agency policies.
  • Demonstrate competence in nursing bag techniques and utilize basic medical equipment.

TREATMENTS AND PROCEDURES:
The Licensed Practical Nurse, with the necessary education and proven clinical skills, may perform specific treatments and procedures as permitted by the Home Health Agency's policies and state regulations. Only advanced LPNs with IV certifications are authorized to perform IV-related procedures.

POSITION QUALIFICATIONS:
  • IV certification.
  • Strong verbal and written communication skills.
  • Ability to access clients' homes, which may not be wheelchair accessible.
  • Physical abilities must include sufficient hearing, eyesight, and dexterity to conduct assessments and provide care.
  • Physical activities may involve walking, sitting, bending, standing, and lifting clients.
  • Graduation from an accredited Practical Nursing program.
  • Current licensure as a Licensed Practical Nurse in the state.
  • One or more years of experience in an acute/sub-acute care setting; home health experience preferred.
