Job Summary

Pinnacle Home Care Inc. is seeking a skilled Occupational Therapist to join our team of healthcare professionals. As an Occupational Therapist, you will be responsible for providing high-quality care to patients in their homes, promoting independence and improving functional abilities.

Key Responsibilities
  • Evaluate patients' functional status, identifying areas for improvement and developing personalized treatment plans.
  • Conduct home assessments to identify hazards and barriers to independent living, recommending modifications and equipment as needed.
  • Develop and implement treatment programs, setting goals for improved function and communicating with referring physicians and other healthcare professionals.
  • Teach patients new skills and retrain them in daily activities lost due to illness or injury, in accordance with organizational policy.
  • Fabricate splints and instruct patients in the use of adaptive equipment to improve function.
  • Train patients in the use of prosthetic and/or orthotic devices.
  • Maintain accurate clinical records, clinical notes, and reports, submitting them in accordance with organizational policy.
  • Collaborate with other healthcare professionals to ensure coordinated and comprehensive care.
  • Identify patient and family/caregiver needs for other home health services and refer as appropriate.
  • Instruct patients' family/caregivers and other healthcare personnel in treatment regimes when indicated and appropriate.
  • Supervise certified occupational therapy assistants according to organizational policy and state regulations.
  • Participate in discharge planning for patients.
Requirements
  • Graduate of an Occupational Therapy curriculum accredited jointly by the Committee on Allied Health Education and Accreditation of the American Medical Association and the American Occupational Therapy Association.
  • Registered with the National Registration Examination of the American Occupational Therapy Association.
  • Licensed to practice as an OT in the State of Florida.
  • Two years of clinical experience as an occupational therapist.
  • Current BLS Certification.
  • Demonstrates good verbal and written communication and organization skills.
  • Licensed driver with an automobile that is in good working order and insured in accordance with the organization's requirements.
  • This position requires that the employee is willing and able to meet the physical requirements as necessary.
