About the Role

In this role, you will be responsible for conducting client assessments, developing personalized care plans, and providing ongoing training and supervision to our care staff. You will also be part of an on-call rotation, ensuring that our clients receive continuous support and care.

Key Responsibilities

  • Conduct thorough client assessments to identify individual needs and develop tailored care plans
  • Provide ongoing training and supervision to care staff to ensure they are equipped to deliver high-quality care
  • Participate in on-call rotations to provide emergency support and care to clients
  • Maintain accurate records of client interactions and care provided

Requirements

To be successful in this role, you will need:

  • A current LPN license in the Commonwealth of Massachusetts
  • Current TB or Chest X-ray; Current CPR Certification
  • Supervisory experience preferred
  • Compassionate and empathetic approach to care
  • Excellent communication and interpersonal skills
