About the Role

We are seeking a highly skilled Medical Care Professional to join our team as a Senior Health Specialist at Abode Care Partners. In this role, you will provide compassionate, evidence-based medical care to seniors and patients with complex needs.

Key Responsibilities
  • Examine and treat chronic and urgent episodic illnesses, minor injuries, accidents, and other injuries or illnesses in various living settings.
  • Collaborate with caregivers and other healthcare professionals in providing interdisciplinary care and maintaining a comprehensive plan of care.
  • Order and interpret laboratory and diagnostic testing as needed.
  • Provide referrals to other healthcare practitioners when necessary.
Requirements
  • A master's degree in nursing (MSN) and an active state Nurse Practitioners license.
  • Current Nurse Practitioner National Board Certification and active state DEA licensure.
  • At least five years of nursing experience, with at least two years of nurse practitioner experience preferred.
  • Strong interpersonal communication skills – verbal and written.
  • Excellent time management skills with accountability for practitioner's daily workflow.
