About the Role

We are seeking a highly skilled Registered Nurse to join our team as a Patient Care Coordinator. This role offers a competitive salary of $ $65,000 - $85,000 per year, depending on experience, plus comprehensive benefits.

Job Description

The Patient Care Coordinator will provide direct nursing care in accordance with established policies and protocols. Responsibilities include implementing and monitoring patient care plans, monitoring, recording, and communicating patient condition, serving as a primary coordinator of all disciplines for well-coordinated patient care, noting and carrying out physician and nursing orders, and assessing and coordinating patient discharge planning needs.

Required Skills and Qualifications

  • Licensure / Certification / Registration:
    • BLS Provider obtained within 1 month of hire date or job transfer date required.
    • Licensed Registered Nurse credentialed from the Texas Board of Nursing required.
  • Education:
    • Diploma from an accredited school/college of nursing required OR a bachelor's level degree not in nursing, an active RN License, and enrolled in an Alternate Entry Masters of Science in Nursing Program with a minimum of 1 year in the program completed.
  • Additional Preferences:
    • One year of RN experience preferred.
