Job Summary

We are seeking a dedicated Clinical Nurse Care Coordinator to join our team at Central Health. In this role, you will play a vital part in ensuring seamless transitions of care for our patients.

About the Role

The Clinical Nurse Care Coordinator will work closely with patients, families, and healthcare teams to coordinate care and support patients throughout their hospital stay and post-discharge. This includes developing patient-centered discharge plans, facilitating communication among healthcare providers, and providing education to patients and caregivers on medication management, condition management, and follow-up care. Additionally, you will work collaboratively with our case management team to identify high-risk patients and develop strategies to prevent readmissions.

Key Responsibilities

* Develop and implement individualized care plans that address patient needs and goals
* Collaborate with patients, families, and healthcare providers to ensure effective communication and coordination of care
* Identify patients at high risk for readmission and develop targeted interventions to prevent readmissions
* Provide education and support to patients and caregivers on disease management, medication adherence, and self-care
* Work closely with the care coordination team to facilitate timely discharges and transfers
* Develop and maintain accurate and up-to-date patient records and documentation

Requirements

* Bachelor's degree in Nursing (BSN) or related field required
* Minimum two years of experience in a primary care setting or ambulatory clinic, preferably with focus on patient navigation and transitions of care
* Strong knowledge of clinical quality improvement principles, workflow development, and staff and patient education
* Excellent communication, interpersonal, and problem-solving skills
* Ability to work effectively in a fast-paced environment and prioritize tasks accordingly
* Certification as a Registered Nurse in the State of Texas and Basic Life Support (BLS) certification required

What We Offer

We offer a competitive salary of $85,000 - $105,000 per year, depending on experience, plus a comprehensive benefits package, including medical, dental, and vision insurance, 401(k) retirement plan, and paid time off.
