About the Role

We are seeking a highly skilled and compassionate Patient Care Manager to join our team at LHC Group. As a Patient Care Manager, you will play a critical role in the supervision and coordination of clinical services, ensuring that patients receive high-quality care in the comfort of their own homes.

Key Responsibilities
  • Supervise and Coordinate Clinical Services: Oversee the delivery of clinical services to patients, ensuring that care is provided in accordance with physician orders and agency policies.
  • Provide Clinical Leadership: Lead and direct a team of clinicians, providing guidance and support to ensure that patients receive the best possible care.
  • Coordinate Patient Care: Work closely with patients, families, and healthcare teams to develop and implement individualized care plans that meet the unique needs of each patient.
  • Ensure Quality and Compliance: Monitor and evaluate the quality of care provided, ensuring that it meets regulatory requirements and agency standards.
Requirements
  • Current RN Licensure: Possess a current and active RN license in the state of practice.
  • Clinical Experience: Have a minimum of one year of prior professional nursing experience, preferably in a home health setting.
  • Leadership Skills: Demonstrate strong leadership and communication skills, with the ability to motivate and guide a team of clinicians.
  • Technical Skills: Possess strong technical skills, including proficiency in electronic medical records and other relevant software systems.
