We are seeking a skilled and compassionate Registered Nurse (RN) to join our team as a Home Health Care Specialist. As a key member of our care team, you will provide high-quality, patient-centered care to individuals in the comfort of their own homes.
Responsibilities- Coordinate and oversee all aspects of patients' home care, ensuring seamless transitions and optimal outcomes.
- Collaborate with patient care teams to develop and implement comprehensive care plans that address the unique needs of each patient.
- Educate patients and their families on condition management, treatment options, and self-care techniques.
- Perform thorough physical assessments, vital sign checks, and medication management to ensure accurate and timely care.
- Administer injections, IV therapy, and tube feedings as prescribed by physicians.
- Manage catheters, complete documentation, and supervise LPNs and HHAs as needed.
- Regularly communicate patient progress to clinical managers, physicians, and care teams to ensure continuity of care.
- One (1+) year of clinical experience as a Registered Nurse (RN) or approval by VP Clinical for less than 1 year of experience.
- Current RN license, specific to the state(s) assigned to work.
- Current CPR certification.
- Valid driver's license, reliable transportation, and liability insurance.
