Position Overview

Role Summary: The Case Manager will be responsible for coordinating and delivering services for young individuals in a residential care environment. This role encompasses various responsibilities, including managing medical, educational, and employment-related appointments, as well as facilitating family interactions. The Case Manager will serve as a crucial link between the organization and external agencies involved in the clients' welfare.

Key Responsibilities:

  • Manage a caseload focused on addressing the environmental and psychosocial needs of clients.
  • Offer casework and counseling support to youth and their families regarding their permanency plans.
  • Organize and oversee family visits and interactions.
  • Input and maintain accurate client data in the management system.
  • Engage in case consultations with supervisors and team members.
  • Fulfill various case management tasks, including court preparation, administrative reviews, and report writing.
  • Stay informed about job-related competencies and client-specific needs.
  • Participate in Continuous Quality Improvement initiatives.
  • Contribute to team meetings and case discussions.
  • Collaboratively assess and plan treatment strategies for children facing challenges.
  • Work alongside medical professionals during consultations and assessments.
  • Recognize and utilize community resources to meet the needs of children and families.

Qualifications:

  • Bachelor's degree in Human Services or a related discipline, with one to three years of relevant experience.
  • Completion of all required exams and certifications.
  • Possession of a valid driver's license and a reliable, insured vehicle.
  • Strong awareness of children's needs and the ability to engage effectively with adults.
  • Capability to organize tasks and generate necessary reports.
  • Willingness to work flexible hours, including evenings and weekends, as required.
  • Basic proficiency in computer applications, particularly Microsoft Word.

Salary Range: $47,400-$50,900
