About the Role

We are seeking a highly skilled and compassionate Social Services Director/Admissions Director to join our team at Clayton Center in Clayton, NM. As a key member of our interdisciplinary team, you will play a vital role in promoting and protecting patient rights, dignity, independence, and psychological well-being.

Key Responsibilities
  • Collaborate with social services staff, interdisciplinary team, and administration to ensure comprehensive Psychosocial Assessments are completed for each patient, identifying social, emotional, psychological needs, and strengths.
  • Maintain good working relationships with center employees, meeting with administration and nursing staff to collaborate and plan.
  • Provide therapeutic interventions to assist patients in coping with their transition and adjustment to a nursing center and address behavior or mood problems.
  • Educate patients and families regarding their rights and responsibilities, health care decision making, advance directives, effective problem solving, and available community resources.
Requirements
  • Must possess certifications/licensures as required by the state of employment to practice in long-term care.
  • At least 1-3 years of supervised Social Services experience in a healthcare setting is required.
  • Additional certification in Geriatric Case Management, Hospice & Palliative Care, Gerontology, Clinical Social Work, Health Care, Nephrology, Mental Health, and/or Substance Abuse is preferred.
  • Management/administrative/supervisory experience is preferred.
