Overview of the Care Coordinator Role

The Care Coordinator position is essential for delivering care coordination services to individuals qualified for 1915(i) programs as designated by the North Carolina Department of Health and Human Services. This role encompasses proactive engagement, evaluation, and management of care to guarantee that members access the necessary services. The Care Coordinator works closely with the care team, performs NC Medicaid 1915i Assessments, and formulates individualized care plans tailored to each member's needs.

Primary Responsibilities:

  • Execute NC Medicaid 1915i Assessments to determine eligibility and needs
  • Create and implement person-centered care plans
  • Engage in interdisciplinary care team meetings to enhance service delivery
  • Facilitate transitional care management for seamless service transitions
  • Oversee service provision and ensure adherence to compliance standards

Essential Qualifications:

  • Bachelor's degree in a relevant discipline
  • A minimum of 2 years of experience working directly with individuals experiencing behavioral health conditions, intellectual/developmental disabilities, or traumatic brain injuries
  • Preferred experience in long-term services and supports (LTSS) coordination

Physical Demands:
