About the Role

NC Elder Care is seeking a skilled Nurse Practitioner or Physician Assistant to provide primary care to patients in various long-term care settings. As a key member of our team, you will collaborate with facility staff and primary care/specialty providers to deliver high-quality care on site.

Responsibilities
  • Provide primary care for patients in assigned long-term care facilities
  • Work collaboratively with primary care/specialty providers to provide the best care possible
  • Work collaboratively with facility staff and families
  • Provide clear medical documentation of care provided, either written or within the EHR
  • May be required to participate in on-call/after-hours call
Qualifications and Education Requirements
  • Certified Nurse Practitioner (ANP, GNP, FNP, AGNP) through a national board (ANCC, AANP)
  • Physician Assistant license
  • Minimum of master's degree in Nursing
  • Current unrestricted license (RN & NP) or the ability to obtain in state(s) in which practicing
  • Unrestricted DEA (license to prescribe narcotics) per state regulations (unless prohibited in state of practice) or the ability to obtain
  • Access to reliable transportation that will enable you to travel to client and/or patient sites within a designated area
Preferred Skills
  • Long-term care experience in a variety of settings (SNF, ALF, group homes, CCRC, etc)
  • Proficient with computer systems
  • Understanding of geriatrics, chronic illness, and advanced/terminal illnesses
