We are seeking a highly skilled and compassionate Primary Care Provider to join our team at NC Elder Care. As a Nurse Practitioner or Physician Assistant, you will play a vital role in delivering high-quality care to our patients in various long-term care settings.
Key Responsibilities- Provide primary care to patients in assigned long-term care facilities
- Collaborate with primary care/specialty providers to ensure comprehensive care
- Work closely with facility staff and families to address complex patient needs
- Document care provided clearly and accurately
- Participate in on-call/after-hours call as required
- Certified Nurse Practitioner (ANP, GNP, FNP, AGNP) through a national board (ANCC, AANP)
- Physician Assistant license
- Minimum of master's degree in Nursing
- Current unrestricted license (RN & NP) or ability to obtain in state(s) of practice
- Unrestricted DEA license to prescribe narcotics or ability to obtain
- Access to reliable transportation for travel to client and patient sites
- Long-term care experience in various settings (SNF, ALF, group homes, CCRC, etc)
- Proficiency with computer systems
- Understanding of geriatrics, chronic illness, and advanced/terminal illnesses
