About the Role

We are seeking a highly skilled and compassionate Primary Care Provider to join our team at NC Elder Care. As a Nurse Practitioner or Physician Assistant, you will play a vital role in delivering high-quality care to our patients in various long-term care settings.

Key Responsibilities
  • Provide primary care to patients in assigned long-term care facilities
  • Collaborate with primary care/specialty providers to ensure comprehensive care
  • Work closely with facility staff and families to address complex patient needs
  • Document care provided clearly and accurately
  • Participate in on-call/after-hours call as required
Qualifications and Requirements
  • Certified Nurse Practitioner (ANP, GNP, FNP, AGNP) through a national board (ANCC, AANP)
  • Physician Assistant license
  • Minimum of master's degree in Nursing
  • Current unrestricted license (RN & NP) or ability to obtain in state(s) of practice
  • Unrestricted DEA license to prescribe narcotics or ability to obtain
  • Access to reliable transportation for travel to client and patient sites
Preferred Skills
  • Long-term care experience in various settings (SNF, ALF, group homes, CCRC, etc)
  • Proficiency with computer systems
  • Understanding of geriatrics, chronic illness, and advanced/terminal illnesses
