Position Overview:
We are seeking a Senior Clinical Care Supervisor to lead our operations. This role involves overseeing the daily functions of the hospice site, managing clinical aspects of the program, and ensuring high-quality care for our patients. You will also supervise and guide the performance of your team members.

  • Oversee, evaluate, and direct the activities of designated team members, including assigning admission and evaluation visits, scheduling, and ensuring timely completion of visits.
  • Act as the site manager in the absence of higher management.
  • Supervise clinical and administrative record-keeping processes.
  • Respond to inquiries and referrals with professionalism and empathy.
  • Participate in the on-call nursing schedule and provide direct nursing care in patients' homes.

Qualifications:

  • Must possess a nursing degree and be a Registered Nurse with an ASN or BSN from an accredited institution, along with at least one year of RN experience.
  • A minimum of three years in direct patient care and one year in a management role is required, with two or more years in hospice or home care preferred.
  • Proficient in using nursing assessment tools and office technology, including computers and tablets.
  • Demonstrated sensitivity to the needs of terminally ill patients and their families.
  • Comprehensive understanding of hospice care principles and the needs of terminally ill patients and their families.

Licenses and Certifications:

  • Current nursing license in the relevant state.
  • CPR Certification.
  • Valid driver's license and current automobile insurance.

  • Empathy towards terminally ill patients and their families, demonstrating flexibility in handling various tasks efficiently.
  • Ability to communicate effectively with individuals from diverse backgrounds.
  • Capacity to influence opinions and attitudes positively.
  • Exhibit patient advocacy and responsible workplace behavior, showing maturity, cooperation, and tact.

Specialized Knowledge and Skills:

  • Thorough understanding of hospice care and the roles of various disciplines involved in providing services.
  • Excellent patient assessment capabilities and strong communication skills.
  • Solid knowledge of managed care principles and regulatory guidelines.
