Position Overview:
We are seeking a Senior Clinical Care Coordinator for our Baltimore MD branch. This role involves direct reporting to the Executive Director.

Your responsibilities will encompass the daily management of your designated hospice location, overseeing the clinical elements of the hospice program, and ensuring high-quality care delivery to patients.

You will also be tasked with supervising, guiding, and directing the performance of assigned team members.

Key responsibilities include:
  • Overseeing and managing the performance of designated team members, including assigning admission and evaluation visits.
  • Scheduling visits and ensuring timely completion by staff.
  • Assuming management of the site in the absence of the Executive Director or Administrator.
  • Overseeing clinical, administrative, and recordkeeping functions.
  • Responding to phone inquiries and referrals in a professional and compassionate manner.
  • Participating in the on-call nursing schedule and providing direct skilled nursing care in patients' homes.


Candidate Profile:

Education and Experience:
A degree in nursing is required.

Must be a Registered Nurse with an ASN or BSN from an accredited nursing institution, with at least one year of experience as an RN (licensed in the state of employment).

Specific state requirements may apply.

Three years of direct patient care experience is necessary, along with one year of management experience. A minimum of two years in hospice or home care is required, unless otherwise specified by state regulations.

Proficiency in using nursing assessment tools and office equipment, including computers or tablets, is essential.

Must possess sensitivity towards the feelings of terminally ill patients and their families both before and after a patient's passing.

Understanding of the hospice concept and the needs of terminally ill patients and their families is crucial.

Licenses, Certifications, and Registrations:
Current nursing license in the state of employment is mandatory.

CPR Certification is required.

Must have current automobile insurance and a valid driver's license.

Personal Attributes and Skills:
Demonstrated sensitivity to the emotions of terminally ill patients and their families before and after a patient's death is essential.

Ability to adapt by performing various tasks efficiently and maintaining composure under changing circumstances.

Effective communication with individuals from diverse socioeconomic backgrounds is necessary. The ability to influence opinions and make informed decisions based on judgmental criteria is vital. Evidence of patient advocacy is expected.

Exhibit responsible behavior in the workplace, demonstrating maturity, cooperation, flexibility, and tact when interacting with colleagues and all members of the hospice team.


Specialized Knowledge and Skills:


In-depth knowledge of terminally ill patients and their families, along with a solid understanding of the hospice model; familiarity with the roles of all disciplines providing hospice services; excellent patient assessment capabilities; strong oral and written communication skills; comprehensive understanding of managed care principles and regulatory guidelines (e.g., Medicare, Medicaid, and human resource management principles).
